This is the second page of Marion Trykoski's death certificate. Additional clinical information is hand written on the left margin by Dr. McWilliam, House Resident: "Passed (from) Primary, non-determinate (meningitis); started with pain in ear 6 weeks ago; no redness of eardrum, no discharge; mastoidectomy was exploratory". Undertaker scrolled "To Cutchogue" on the certificate (it is written upside down).
